[MINC-development] max_buffer_size_in_kb

We all now have machines with more than 64MB of RAM.

Would anyone be against me upping the default limit (4MB in most tools)?

In larger files upping this to something much larger (4096000 for
example) will result in much faster runtimes for anything that uses
voxel_loop (minccalc, mincmath, mincreshape, etc).

Perhaps we should take an ENV var approach?
